UPlan Medica Fit Choices

About UPlan Fit Choices

Recreational Sports members enrolled in the UPlan Medica Fit Choices program are eligible to receive a $20-a-month reimbursement for membership fees. The new UPlan Medica Fit Choices program, offered through the Office of Human Resources, rewards members for using the University Recreation Center and St. Paul Gymnasium at least 8 times per month. A maximum of two qualifying adults per household may enroll in the program.

Reimbursement Process

Monthly $20 reimbursements will be directly deposited (via an electronic funds transfer) into qualifying members' personal checking or savings accounts. There will be a two month delay between the month during which a member exercises and the month in which he or she receives the reward. For example, members completing the required number of visits in February will have their participation validated in March and will have their accounts credited in late March or early April. If a member fails to visit eight times during a month, the payment of future rewards will adhere to this schedule as well.

Eligibility

To be eligible for the UPlan Medica Fit Choices program at the Department of Recreational Sports, you must:

Step One

Purchase a Recreational Sports staff/faculty or spouse membership by visiting either the University Recreation Center Member Service Office or the St. Paul Gymnasium Front Desk

Step Two

Visit the UPlan Medica Fit Choices enrollment page by clicking on the "Enroll Here" button below.

Upon enrolling, be prepared to provide the following:

  • Name, contact information, and date of birth
  • University Employee ID number
  • 2012 UPlan health care information
  • Checking or savings account routing and account numbers
    Generally found on the bottom of a check or deposit slip that is used by financial institutions to transfer funds and complete transactions.

Fit Choices Member Login

Due to continuous changes in health care plans on campus, it is important that all current members login to their accounts and ensure that their 2013 Medica information is up-to-date and accurate in the system. Please follow the steps below to accomplish this:

  • Click on the Fit Choices Member Login button below.
  • Use your 2012 insurance information to login (but have your 2013 insurance information nearby for making account changes).
  • Enter your Employee ID number or Rec Sports ID number.
  • Enter your Group ID.
  • Enter your Subscriber ID.
  • Click Next.
  • Click Edit Health Plan Information.
  • Make the necessary changes and double check your numbers.

Member Usage Tracking

Want to track how many times you've visited? Rec Sports handles all participant tracking by sending monthly reports to the health care providers. To find out how many times you've visited during a month, contact Sue Kelzer, Rec Sports Senior Office Assistant, at 612-625-1548 or email kelze001@umn.edu.

All banking information storage, insurance information storage and payment processing is being performed by Vanco Services, LLC, a trusted third party vendor, PCI compliant electronic payment processor located in the Twin Cities.
